Voluntary throughcare case management support for young people exiting the residential component of Mac River Centre (offered to all residents dependent on length of stay). Also offered as a voluntary support to referred (accepted) young people awaiting entry to residential services of Mac River Centre on an identified 'needs' basis.
Provision of service through a tailored mix of telephone, online and in person support, generally for a period of up to 12 weeks although timeframes are negotiable on a case by case basis
Who we assist
Young people 12-18 years involved in the youth justice system who access the services of Mac River Centre Residential Rehabilitation Program.
Eligibility criteria
Client of Mac River Centre Residential Rehabilitation Program.
Referral process
All referrals for Mac River Centre Residential Rehabilitation Program are received via Youth Justice or Youth on Track NSW
