The Toward Home - Prevent service works with tenants and other household members who are at risk of homelessness through coordinated support services to develop goal-oriented personal support plans and to enable them to access services to find and sustain tenancies.

Eligibility criteria
People at risk of homelessness based in the Adelaide Hills, CBD and Southern Adelaide regions
Referral process
Self-referral, referral from community organisations and gateway services
