About
Community Living Supports (CLS) is a community-based mental health psychosocial recovery service offering flexible support to adults who live with a severe mental illness.
We actively promote and work within a trauma informed and culturally responsive lens, offering holistic services where the participant is the lead of their supports.
CLS works with people on their recovery journey, listening to their needs, encouraging independence, developing skills and promoting self-esteem whilst focusing on individual strengths.
Who we assist
People living with a severe and persistent mental illness.
Eligibility criteria
Adults (at least16 years old) with a diagnosed severe mental illness who require a high level of psychosocial support.
Participants must be:
• willing to engage in the program and work on their recovery goals
• living in the local health district catchment area
• linked in with clinical support for their mental health
• Participants must not be accessing another program that provides the same support
CLS does not require a formal diagnosis of a severe mental health condition for Aboriginal people, people aged 16-24, refugees, or people seeking asylum.
Referral process
CLS offers a ‘no wrong door’ approach to ensure accessibility from multiple points of entry. Referrals to CLS often come from local health districts (LHDs), GPs and private clinicians, the department of communities and justice (DCJ), self-referral, family members, and other organisations.
Continued collaboration with clinical support helps us to appropriately address the needs of the people we work with.
