The HMAS will support and link people experiencing homelessness, at risk of homelessness and people from multiculturally diverse (CALD) backgrounds to primary care. The HMAS will facilitate continuous care and support by connecting people experiencing homelessness, at risk of homelessness and those from multiculturally diverse backgrounds to community-based primary care providers and General Practitioners well-suited to their unique needs. The HMAS will deliver onsite services at Mission Australia locations and through co-located facilities. The service will provide immediate assistance, assessments, education, and referrals to primary care and additional agencies as needed.

Who we assist

The program is to support hard to reach individuals who are currently not accessing allied health services (such as GP's). People who identify as culturally or linguistically diverse and/or identify as multicultural. People who identify as experiencing homelessness.

Eligibility criteria

Living in the North Sydney PHN catchment area.
