Home and Healthy Sydney and South East Sydney


The Home and Healthy program helps adults at risk of, or experiencing, homelessness as they exit health settings. We offer practical support and guidance to help secure and maintain housing, and work towards other goals such as employment, training and community connection.

As of June 2021, Home and Healthy is no longer accepting new referrals. The NSW Government and Mission Australia will work together over the next few months to transition existing service users to other support services currently operating in central Sydney. Health staff with patients who are at risk of or experiencing homelessness on exit from health facilities are advised to contact the Department of Communities and Justice local housing services.

Who we assist

aged 18-65 years
at risk of or experiencing homelessness
leaving hospital or engaged with community or drug health in the Sydney, South East Sydney and St Vincent’s Local Health Network and
able to live independently with external support

Eligibility criteria

At risk of or experiencing homelessness
Aged between 18 and 65 years
Engaged with an agreed health facility in Sydney Local time of referral

Referral process

Closed Referral Pathways. Referrals are triaged within each Local Health District and then forwarded to Home and Healthy for arranging Intake Assessment.
