Deliver intensive case management for young people receiving or have received treatment at the MYAHC. Lead and facilitate an assessment, referral and case management.
Work with housing and homelessness sector, including the department to connect young people accessing MYAHC to relevant products, services and available funding.
Undertake assessments of the young person’s housing and support needs and ensure risk assessments and safety planning is completed
Provide case management support to assist the young person to access housing and relevant support services
Advocate and facilitate access to housing, SHS and any other mainstream services
Support the young person’s transition into housing, including private rental options.

Who we assist

Young people experiencing or at risk of homelessness or are living in inappropriate or unsafe housing
Over school leaving age and between 16-25 years.

Referrals from MYAHC - various departments. Program still in the design phase.
