The AOD Continuing Coordinated Care (CCC) Service in Far West NSW provides care coordination and wraparound services for people with a substance use disorder who are receiving community-based treatment, while also experiencing a high level of need. CCC supports clients with intensive support to maintain engagement with treatment services, including maintaining linkages to AOD and primary health services; functional living supports; and support to maintain community and family connections.

Who we assist

Adults with a substance use disorder and complex needs

Eligibility criteria

Have recently been assessed by a Drug & Alcohol clinician as having a substance use disorder; and
are actively receiving or have recently received (in the previous month) an AOD treatment intervention; and
have a care plan in place that identifies significant additional needs that cannot be addressed by standard AOD treatment and that may jeopardies their engagement with treatment.
