Who we assist
Identified tenants that may need additional supports/ referrals or connections to increase capacity and ensure they are able to sustain a tenancy. This may include people with disability or severe and chronic mental health illness, elderly people facing vulnerability, carers with long term caring responsibilities, and those impacted by substance use.
Eligibility criteria
-You must be a current tenant residing within Social, Affordable or Private Market properties within the Midtown Community MacPark
-Tenants may require additional support/referral/connection to external services to enhance independent living skills, and enable Community connectedness. Support areas may include connecting with mental/physical health providers, employment/educational options, AOD referrals, financial counselling, social support groups etc.
Referral process
Self referrals via the team located onsite at MacPark, Mon-Fri.
The TSCS referral form can be completed by an MAH team member when concerns have been identified and external support services can also complete the referral form and return to the email for the CS Midtown team. Referrals are flagged and reviewed weekly at a JRC meeting with both Mission Australia Housing (MAH) and Mission Australia Community Services (CS). If accepted the tenant is contacted directly by the Support Coordinator.
