Delivery of psychosocial support in partnership with Clinical supports delivered by the Local Health District to people with severe mental illness to maximise their participation in the community. Flexible support including attending appointments, community participation, facilitate access to education, training and employment, Medication compliance, Daily Living Skills including shopping, budgeting, transport and self-care. Supporting clients through the transition from hospital discharge to return to the community.

Who we assist

Clients with severe mental health issues and very high and complex needs, people living in social housing, people in community detention, people exiting correctional facilities, Aboriginal people, refugees, boarding house residents. No age limitations.
