Triple Care Farm Aftercare

Alcohol & Other Drugs

Voluntary throughcare case management support for young people exiting the residential component of Triple Care Farm (offered to all residents dependent on length of stay). Also offered as a voluntary support to referred (accepted) young people awaiting entry to residential services of Triple Care Farm on an identified 'needs' basis.
Provision of service through a tailored mix of telephone, online and in person support, generally for a period of up to 12 weeks although timeframes are negotiable on a case by case basis

Who we assist

The service works with young people aged between 16 and 24 years who access the Triple Care Farm Residential Rehabilitation program.

Eligibility criteria

Client of Triple Care Farm Residential Rehabilitation.

Referral process

We accept self-referrals and referrals by other agencies or guardians via an application form, followed by phone and face to face assessment.
