Mental Health Recovery

Community Living Supports (CLS) provides recovery-oriented psychosocial supports to complement clinical support to people aged 16+ who are impacted by mental health conditions. In close collaboration with NSW health, housing providers and other relevant stakeholders, support includes transition from hospital or correctional facilities to community living. Within a recovery framework, CLS delivers capacity building in daily living skills, access to the community and clinical assistance, links to education, vocational training and employment. Participation in social, community access, leisure and recreational activities is central to support.

Who we assist

People living with a severe and persistent mental illness.

Eligibility criteria

-Diagnosed with and impacted by mental health condition
-Transitioning from either correctional facility or hospital
- Is aged 16 and above (no upper age limite, however, the program provides mental health related support only)
-Has appropriate clinical mental health support in place
- Has a willingness and capacity to engage in support
- Has given informed consent
-Lives in the catchment areas of St George, Sutherland, Prince of Wales or St Vincent's health networks.

Referral process

Referrals mainly come from NSW Health or the Department of Communities and Justice Health.
Referrals may come from other CMOs, housing providers, self, family or carers as long as there is appropriate clinical mental health support involved in the client's care. Referral email: [email protected]
