Who we assist

Homeless men and women, those at risk of homelessness and those experiencing financial or social disadvantage who have been approved for residential aged care by an Aged Care Assessment Team (ACAT). A high proportion of residents suffer from mental health and/or drug and alcohol related illnesses. They require 24 hour nursing and medical care, support with activities of daily living, medication administration, nutrition and social activities. We also encourage applications from those who identify as being Aboriginal and/or Torres Strait Islander, Culturally or Linguistically Diverse, or LGBTIQA.

Eligibility criteria

Prior to admission all potential residents must be assessed by the Aged Care Assessment Team (ACAT) as being eligible for admission to a residential aged care facility. This is based on the care needs of the individual.

As Mission Australia residential aged care facilities accommodate the homeless, those at risk of homelessness, and those experiencing financial or social disadvantage, the potential resident must complete a Department of Human Services SA457 form to validate their eligibility.

Referral process

Referrals come through local hospitals, outreach services, General Practitioners, crisis accommodation providers, Public Guardian or Trustee, and others. Residents are also able to self-refer, as can family members or friends on their behalf. Please see www.myagedcare.gov.au
