Drug and Alcohol Youth Services - Aftercare provides case coordination and support to young people as they transition from DAYS residential programs including the Youth Withdrawal and Respite Service, the DAYS Residential Rehabilitation Service or THASP. Aftercare works to sustain progress made at the DAYS residential programs and minimise relapse of Alcohol and Other Drugs, as well as engaging young people to address their physical, social and emotional needs. This includes sourcing and maintaining accommodation and participation in vocational, education and training. The Aftercare Serve includes the provision of outreach case coordination and counselling.

Who we assist

Young people between the ages of 12-22 who have engaged in the DAYS Youth Withdrawal and Respite Service, the DAYS Residential Rehabilitation Service or THASP and are leaving the programs to return to the community.

Eligibility criteria

The young person is leaving one of the three DAYS residential programs at any stage of their treatment and is seeking support to sustain progress made at DAYS to minimize relapse of Alcohol and Other Drugs. The young person would also like support to address their physical, social and emotional needs and goals. This could include sourcing/maintaining accommodation, engaging in vocation/education/training, family work, independent living skills, connection to community and financial counselling.

Referral process

Referrals for young people will be through the DAYS Youth Withdrawal and Respite, Residential Rehabilitation and THASP case managers.
