Mission Australia and Sonder are collaborating to support people living with mild to moderate co-occurring, substance use and mental health concerns. PTW offers counselling, care coordination and case management from AOD Workers, A First Nations AOD Worker, Comorbidity Clinicians, Lived Experience and Peer Workers and a Family Caseworker and groups. The service is funded by Adelaide PHN through the Australian Government’s PHN Program and is free to participants.

Who we assist

Partners Toward Wellbeing supports all people 18 years and over and prioritises support to Culturally and Linguistically Diverse communities as well as older persons, families and Aboriginal and Torres Strait Islander people across the Adelaide Metropolitan Region.

Eligibility criteria

All persons over 18 living with co-occurring mental health and alcohol or other drug concerns
