The Tailored Support Coordination Service provides Goal-based support coordination focused on connecting residents with supports and opportunities that will help them achieve their goals to improve their wellbeing and build independence.

Who we assist

People who are at-risk of or currently experiencing homelessness, and/or are at a stage in their life where they are want to build towards housing independence, but are constrained by their current housing situation, and want support to achieve their goals.

Eligibility criteria

Must be on the NSW Housing Pathways Register
Have the capacity to live independently
Express willingness to fully participate in the Transitions Program
Have the ability to engage with education and employment.

Referral process

Complete a referral form, once the referral form has been received, The PM will arrange for an assessment with the client. Once assessed, applicant will go to a committee for approval by MA community services and MAH. If approved the client will go on a waitlist for a property to become available.
