Annie Green Court is a residential aged care facility for 72 people who are homeless or at risk of homelessness, or who have experienced financial or social disadvantage. A range of medical and allied health supports are available to all residents, as well as recreational activities and financial management support. Annie Green Court promotes a consumer-focused high quality and independent lifestyle where each resident has their own furnished room with ensuite, and access to communal dining and lounge rooms shared between only 12 residents in each area. There is also a rooftop garden and lower courtyard. Security systems maintain a safe yet non-restrictive environment.

Who we assist

Aged frail homeless Men and Women, or those at risk of homelessness and financially and socially disadvantaged. The residents age ranges from 45 - 100 and beyond. 86% of the residents suffer from mental illness. They require 24 nursing and medical care, support with activities of daily living, medication administration, nutrition, social activities.

Eligibility criteria

Aged Care Assessment Team need to assess the resident for eligibility to enter aged care facility and we have to receive the Aged Care Client Assessment.

Referral process

Social workers and Case managers in the hospitals and community services, Public Guardians, from another aged care facility and by the client him/herself or family member. The Aged Care Assessment Team must assess the resident before they can enter Aged Care.
