The Health Equity and Access Program (HEAP) is designed to bridge the gaps in Primary Healthcare access for individuals experiencing homelessness and individuals from culturally and linguistically diverse backgrounds (CALD).

HEAP provides assessment, education, and facilitates referrals to General Practitioners, Psychologists, Psychiatrists, and other health and community services.

Our team will facilitate by connecting you to important health services like doctors, nurses, mental health specialists, and other health and community services that will be able to provide you with continuous support.

Who we assist

The Health Equity and Access Program has been designed to assist both people who are experiencing homelessness AND/OR people from a Culturally and Linguistically Diverse (CALD) or a Non-English-Speaking Background (NESB) to access Primary Healthcare.

Eligibility criteria

You are eligible if you:

- Identify as Culturally and Linguistically Diverse (CALD) or are from a Non English-Speaking Background (NESB)

OR

- Are currently experiencing homelessness

To access support from HEAP you must live in The Northern Sydney Health District:

- City of Ryde
- Hornsby Council
- Hunters Hill Council
- Ku-ring-ai Council
- Lane Cove Council
- Mosman Council
- Northern Beaches Council
- North Sydney Council
- Willoughby Council
